Vitamin tests, and what the number is actually tracking
Vitamin D, B12 and folate are among the most commonly requested add-on tests. Each measures something slightly different from what people assume.
Vitamin tests are frequently added to routine panels, either by request or as part of a broad screen. The results are often read as a simple sufficiency score, and the reality is a little more specific in each case.
Vitamin D measures a storage form
The standard test measures 25-hydroxyvitamin D, which is the main circulating form and the best available indicator of overall status. It is not the biologically active form, which is produced from it in the kidneys and circulates at much lower levels.
Measuring the active form is occasionally useful in specific situations and is a poor indicator of general status, because the body maintains it at the expense of stores.
Levels follow a strong seasonal pattern in temperate countries, typically lowest at the end of winter. A result taken in February and one taken in August describe the same person differently, which is why comparing results across seasons requires care.
Supplementation raises the level over weeks to months rather than days, so a result taken shortly after starting a supplement reflects the period before it more than after.
Vitamin B12 and the availability problem
The routine test measures total B12 in serum, most of which is bound to a carrier protein that does not readily deliver it to cells. Only a minority is bound to the carrier that does.
This means the total can look adequate while the fraction actually available to tissues is not. Where the clinical picture and the number disagree, additional tests exist that assess the functional consequences rather than the concentration.
Supplements, including ordinary multivitamins, raise the measured level, which can obscure the picture in someone who has recently started taking them.
Folate: recent intake or stores
Serum folate reflects recent intake to a substantial degree. A folate-rich meal or a supplement in the preceding days raises it, which makes a single serum measurement a weak indicator of longer-term status.
Red cell folate is a better reflection of stores, because it is fixed at the time the cells were made and does not respond to recent intake. It is more expensive and less frequently measured.
Folate and B12 are usually assessed together, because they participate in overlapping processes and because the consequences of low levels look similar in the blood count.
Why they are read alongside the blood count
Both B12 and folate affect the production of red blood cells, and prolonged shortage of either shows up in the mean cell volume before it shows up as symptoms.
This is why a raised mean cell volume on a full blood count frequently prompts these tests, and why the blood count is part of interpreting them rather than a separate matter.
A general caution about broad vitamin panels
Testing a long list of vitamins without a specific question generates results that are difficult to act on, and the arithmetic of reference ranges guarantees that some of them will fall outside their intervals by chance.
Where a specific deficiency is plausible from symptoms, diet or a medicine that affects absorption, a targeted test answers a clear question. That is a different activity from a broad screen, and produces a much more interpretable result.
